Step-by-Step process of an IUI procedure: What to expect

Intrauterine Insemination (IUI) is one of the most commonly recommended fertility treatments, especially for couples facing unexplained infertility, mild male factor infertility, or ovulation-related issues. For many, the thought of undergoing a fertility procedure comes with plenty of questions — and one of the most frequent is: What exactly happens during the procedure?”

This guide explains each step of the IUI process, so you know exactly what to expect — from initial evaluation to post-procedure care.

What is IUI?

Intrauterine Insemination (IUI) is a simple, non-invasive fertility treatment where sperm is directly placed into a woman’s uterus at the time of ovulation to increase the chances of fertilization. The aim is to bring the sperm closer to the egg and bypass obstacles like cervical mucus or low motility.

Who can benefit from IUI?

Step-by-Step Process of the IUI Procedure

1. Initial consultation & Evaluation

Your IUI journey begins with a thorough fertility consultation. This includes:

  • Reviewing medical history.

  • Pelvic ultrasound to assess ovarian reserve and uterine health.

  • Hormonal blood tests.

  • Semen analysis to check sperm count, motility, and morphology.

2. Ovulation monitoring and Cycle planning

Depending on your cycle and medical condition, IUI can be done:

  • In a natural cycle, or

  • With ovulation-stimulating medications (Clomiphene Citrate, Letrozole, or gonadotropins)

We closely monitor follicular development via ultrasound and track hormone levels (like LH or estradiol) to pinpoint the ideal time for insemination.

3. Triggering ovulation

Once the dominant follicle reaches the optimal size (usually 18–20mm), an HCG injection (trigger shot) may be given to stimulate ovulation. IUI is typically scheduled 34-40 hours after the trigger, which is the best window for fertilization.

4. Semen Collection and Preparation

On the day of IUI:

  • Your partner (or donor) provides a fresh semen sample.

  • The sample is “washed” and prepared in the lab — this process concentrates the healthiest, most motile sperm and removes seminal fluid or debris that may cause cramping.

5. The IUI procedure

This is a quick, painless procedure done in our clinic. Here’s what happens:

  • You lie down in a gynecological position.

  • A thin, flexible catheter is inserted through the cervix into the uterus.

  • The prepared sperm is slowly injected.

  • The entire process takes just a few minutes and is usually painless, though some may feel mild cramping.

You rest for a few minutes afterward, and then you can resume normal activities.

6. Post-IUI care & Instructions

After the procedure:

  • Light spotting or mild cramping is normal.

  • You’ll be advised to avoid heavy physical exertion or intercourse for 24 hours.

  • In some cases, progesterone support (oral or vaginal) is prescribed to help implantation.

7. Pregnancy testing

Pregnancy testing

About 14 days after the IUI, a beta hCG blood test is done to confirm pregnancy. While waiting, we understand the emotional toll and provide emotional support and counseling at every stage.

Success rate of IUI

The success of IUI depends on:

  • Woman’s age.
  • Sperm quality.
  • Cause of infertility.
  • Number of follicles developed.

On average, IUI success rates range from 10–20% per cycle, and rates may increase with 2–3 attempts. At Bavishi Fertility Institute, we provide personalized treatment plans to maximize your chances of success.

Conclusion

IUI is a safe, simple, and effective fertility treatment that can be a promising first step on your journey to parenthood. By understanding each stage — from initial evaluation to post-procedure care — you can feel more prepared and confident throughout the process. While the success of IUI can vary based on individual factors, many couples find hope and success through this approach, especially with expert guidance.

IUI is generally safe, but mild cramping, spotting, or in rare cases, infection or multiple pregnancy (if medications are used) can occur.

Yes. Most women resume normal activities immediately after the procedure, though it’s advised to avoid heavy lifting or intense exercise for 24 hours.

There are no specific dietary restrictions, but a balanced, fertility-friendly diet rich in antioxidants, vitamins, and hydration is recommended.

Chronic stress may impact hormonal balance, so managing stress through relaxation, yoga, or counseling may help improve outcomes.
