IUI process explained: What to expect at every step

Intrauterine Insemination (IUI) is often one of the initial and commonly recommended fertility treatments for individuals or couples trying to conceive. It is a simple, less invasive procedure that can significantly improve the chances of pregnancy in many cases. Having a clear understanding of how IUI works, what to expect during the process, and its potential benefits can help patients feel more confident, prepared, and supported as they take their first steps on the path to parenthood.

In this blog, we’ll walk you through the entire IUI procedure—step by step—so you know exactly what to expect at each stage.

What is IUI?

Intrauterine Insemination (IUI) is a fertility treatment in which specially prepared sperm is placed directly into the uterus around the time of ovulation. This increases the chances of fertilization by helping the sperm get closer to the egg.

Who can benefit from IUI?

IUI is commonly recommended for:

  • Mild male infertility.

  • Unexplained infertility.

  • Cervical factor infertility.

  • Ovulation issues.

  • Couples using donor sperm.

  • Same-sex female couples.

Step-by-Step guide to the IUI process

Step 1: Initial consultation and Fertility evaluation

Your IUI journey begins with a comprehensive consultation at the fertility clinic. This includes:

  • Medical history review.

  • Physical examination.

  • Hormonal blood tests (AMH, FSH, LH, TSH, etc.)

  • Ultrasound scan of the ovaries and uterus.

  • Semen analysis for male partner.

Based on the results, our fertility specialists design a personalized treatment plan.

Step 2: Ovulation monitoring or Ovulation induction

Timing is critical in IUI. We either:

  • Monitor your natural ovulation cycle using ultrasound and blood tests
    OR

  • Stimulate ovulation using fertility medications (like Clomiphene Citrate or Gonadotropins) to produce one or more mature eggs.

Throughout this phase, we perform:

  • Follicular scans to track egg development.

  • Hormonal blood tests to monitor ovulation progress.

Step 3: Triggering ovulation

Once your follicle(s) reach the optimal size (usually 18–20 mm), a trigger injection (HCG) is given to induce ovulation. The IUI procedure is then scheduled approximately 36–40 hours after the injection.

Step 4: Sperm collection and Preparation

On the day of the procedure:

  • The male partner provides a semen sample at a clinic.

  • If using donor sperm, we thaw a pre-selected and screened donor sample.

  • The sperm is then washed and concentrated to separate motile, healthy sperm and remove debris or chemicals that could irritate the uterus.

This process increases the chances of fertilization by improving sperm quality and volume.

Step 5: The IUI procedure

The IUI procedure is simple, quick, and usually painless:

  1. You lie on the exam table, similar to a Pap smear.

  2. A thin, flexible catheter is gently inserted through the cervix.

  3. The prepared sperm sample is injected directly into your uterus.

The whole procedure takes just a few minutes. You may rest briefly afterward before returning to your normal routine.

Step 6: Post-IUI instructions & Medications

After the IUI:

  • We may prescribe progesterone supplements to support the uterine lining.

  • You’ll be advised to avoid heavy lifting or intense workouts for a few days.

  • Most women resume normal activities within 24 hours.

Step 7: The two-week wait

This is the most emotionally challenging part for many couples. You’ll wait about 14 days before taking a pregnancy test. During this time:

  • Avoid stress and maintain a healthy lifestyle.

  • Continue taking medications as prescribed.

  • Watch for early pregnancy symptoms (though not everyone will have them).

Step 8: Pregnancy testing & Next steps

After two weeks:

  • A blood test (beta-hCG) is done at our clinic to confirm pregnancy.

  • If positive, we schedule your first pregnancy scan after 10–14 days.

  • If negative, we review your treatment and decide whether to repeat IUI or explore other options like IVF.

FAQs

Most patients report that the IUI procedure is quick and virtually painless. You may feel slight cramping or discomfort similar to a Pap smear.

Doctors usually recommend trying 3–6 IUI cycles before moving to IVF, depending on the woman’s age, cause of infertility, and response to treatment.

Yes. If you ovulate regularly, a natural cycle IUI may be suggested. However, using fertility medications often improves the chances of success.

Sperm washing removes low-motility sperm, debris, and chemicals in the semen, allowing only high-quality sperm to be placed into the uterus, enhancing fertilization chances.

Maintain a balanced diet, avoid alcohol and smoking, reduce stress, and get adequate sleep to support fertility and improve treatment outcomes.

When fertility medications are used, there is a slightly increased chance of multiple pregnancies—usually around 10–20% depending on the medication used.

Most women resume regular activities within a few hours to a day after IUI. However, intense physical activity should be avoided for a couple of days.
