What is IMSI?

IMSI stands for Intracytoplasmic Morphologically Selected Sperm Injection. It’s an advanced version of ICSI. In standard ICSI, sperm is selected at 400x magnification. In IMSI, we use a high-power inverted microscope with digital imaging to magnify sperm up to 6000x-6600x. 

 

At this magnification, embryologists can see tiny defects in the sperm head called vacuoles that are invisible under normal ICSI. These vacuoles are often linked to sperm DNA damage. By choosing sperm without these defects, we aim for better fertilization, better embryo quality, and higher pregnancy rates.

How is IMSI done in the Lab?

At BAVISHI Fertility Institute, IMSI is performed with precision:

  • Semen Preparation: The semen sample is processed using density gradient or swim-up to isolate the best motile sperm.

  • High Magnification Screening: A single drop of prepared sperm is placed in a special glass-bottom dish under a high-resolution inverted microscope. The embryologist scans individual sperm at 6000x using specialized Nomarski optics.

  • Morphological Assessment: Each sperm is checked for head shape, size, symmetry, and most importantly, vacuoles. Sperm with large vacuoles or abnormal heads are discarded.

  • Selection & Injection: The best morphologically normal sperm is picked up with an ICSI needle and injected directly into the egg, just like ICSI.

  • Culture & Monitoring: Fertilized eggs are monitored in the incubator, and embryo development is tracked.

  • High Sperm DNA Fragmentation: IMSI shows clear benefit for men with DFI >30%, with improved clinical pregnancy rates.

  • Recurrent ICSI Failure: Couples with 2 or more failed ICSI cycles with good eggs often benefit from IMSI.

  • General IVF Population: For men with normal semen parameters, IMSI offers no added advantage over ICSI. It’s not for everyone.

ESHRE and ASRM state that IMSI should be used selectively, not routinely.

When should patients use IMSI?

IMSI is beneficial for: 

  • High perm DNA Fragmentation Index (DFI >30%).

  • Severe Teratozoospermia: Very high percentage of abnormal sperm forms.

  • Recurrent ICSI failures: 2+ failed cycles despite good egg quality and embryo development.

  • Recurrent miscarriage: Where male factor DNA damage is suspected.

  • Previous poor embryo quality: With no clear female factor.

When is IMSI not needed?

If semen parameters are normal, DFI is <15%, and it’s your first IVF cycle, standard ICSI gives excellent results. Doing IMSI unnecessarily adds time and cost without extra benefit.
